Description:
Three-month cardiac rehabilitation program designed to improve participants' ability to take control of their heart health, quality of life, and physical functioning. The "Hearts in Motion" program is a vascular risk-reduction, research program aimed at helping Nova Scotians take control of their health. The program includes exercise, healthy eating and education coordinated by a team of health professionals over a 14-week period; follow-up appointments are also made at 6 months and 12 months after the program. "Hearts in Motion" is available at 3 locations in the Capital Health district (Lower Sackville, Dartmouth, and Spryfield).
Referral is required by your doctor or another health professional involved in your care. Eligible participants have vascular disease (such as heart disease, stroke, TIA, peripheral vascular disease) or have significant risk factors (diabetes, high blood pressure, smoking, high cholesterol). Contact for more information. There is no cost to participate.

Communities represented include Woodlawn, Dartmouth Centre, Shannon Park, Wallis Heights, Southdale, Woodside, Montebello, Highfield Park, Portland Estates, Manor Park, Waverley, Crichton Park, Bel Ayr Park, Albro Lake, and Harbourview.
